Our nurses evaluate your condition on each visit and work with your physician to develop a goal-driven perosnalized treatment plan. Our expert care team works to improve your quality of life and prevent unwanted hospital visits on your road to recovery. Our licensed nurses provide compassionate care in the comfort of your own home, including:

·        Medication management, teaching and administration

·        Assessments at each visit on your condition and risk for going to the hospital

Our skilled nurses also measure blood pressure, pulse, respirations, lung sounds, blood glucose or pulse oximetry, as ordered by your doctor.

Activities of Daily Living Support (ADL’s) Activities of daily living (ADLs) include the tasks we do each day to take care of ourselves, like personal hygiene or preparing meals. Qura Home Health Care offers certified home health aides (HHA) who assist patients with daily living activities while they are under the care of the home healthcare team. In-Home ADL Support Includes:

·        Personal care (bathing, dental care, hair care, toileting)

·        Dressing and undressing

·        Transferring to and from bed/chair

·        Light meal preparation

·        Eating

·        Light housekeeping

·        Assisting with basic activities and daily living needs

·        Measuring and recording patient’s temperature, pulse, respiration and blood pressure when ordered and/or directed by the homecare team

Continence Care For those living with incontinence, Qura Home Health Care’s goal is to help patients achieve the highest level of dignity and comfort. Forty percent of women and 25 percent of men over the age of 65 face some degree of incontinence. In-Home Continence Care May Include:

·        Urinary catheter maintenance

·        Moisture management

·        Toileting programs

·        Physical therapy, including pelvic floor muscle exercises

·        Home accessibility assessment

·        Patient coaching and education to help prevent accidents before they occur

Disease Management Our programs aim to help people with chronic conditions achieve better personal health and independence. This system of education, intervention, communication and self-care helps patients take control of their own health and well-being. Qura Home Health Care’s focuses on each of the following conditions:

·        Chronic obstructive pulmonary disease (COPD)

·        Coronary artery bypass graft

Frequency of Visits: During the first few weeks of this program, registered nurses and therapists will visit patients several times per week. As patients learn to manage their own condition, visits will become less frequent.

Nursing Care

Patients are monitored and visited regularly by our home health nurses. During regular in-home visits, nurses will monitor and educate patients to:

·        Follow a prescribed treatment plan

·        Track and monitor their symptoms

·        Take medications as ordered by the and watch for side effects

·        Maintain a heart healthy diet and weight

·        Watch the amount of fluid they consume

·        Make healthy lifestyle choices

Rehabilitation

When prescribed by the physician, patients receive regular visits from our home health therapists. During regular in-home visits, rehabilitation professionals will:

·        Screen patients to determine their level of strength and endurance

·        Asses the patient’s risk of falling and create a prevention plan

·        Develop an individual rehabilitation program to meet the patient’s specific needs

·        Teach patients how to exercise and develop a regular exercise plan

Patient Education

Our home health team teaches patients and their caregivers to use tools to monitor daily activities and record weight and symptoms. By caring for themselves and managing their symptoms, patients take control and improve their quality of life.

Medication Management One of the most critical aspects of recovery is taking medications as directed. Qura Home Health Care’s registered nurses help patients make sure they are taking the right medicine and the right dosage, at the right time, and according to the doctor’s orders. This includes organizing medicines and setting up reminders to keep patients on track.Our nurses partner with each patient’s doctor to make sure medicines are working and controlling symptoms appropriately. If a patient’s health status changes, the nurse will contact the doctor immediately. This allows the patient’s care team to make changes in medication or adjust a doctor’s orders in a timely manner.

Ostomy Care at Home

Qura Home health Care offers knowledgeable and supportive nursing care to help patients who have an ostomy. Specially certified wound, ostomy, and continence care nurses visit patients’ homes to teach them and their caregivers how to care for the ostomy and live comfortably with an ostomy pouch.

Ostomy Education

As part of personalized treatment sessions, Qura Home Health Care’s wound, ostomy, and continence care nurses educate patients and caregivers on how to properly care for an ostomy. This approach helps patients maximize their independence and improve their quality of life. Instructional support includes: 

·        How to empty the pouch and change the pouch

·        How to clean the affected and surrounding area

·        How to care for the skin around the stoma

·        Food guidelines for ostomy patients to minimize discomfort

With an ostomy, waste no longer leaves the body from the anus and instead leaves the body through the part of the intestine at the ostomy opening. This part of the intestine is called the stoma. There’s no muscle around the stoma, so the individual cannot control when waste or gas leaves the body. Waste automatically goes from the stoma into a plastic bag (or “pouch”) around the stoma. This pouch will block the smell of the waste. Qura Home Health skilled nurses teach patients how to care for the ostomy and change the pouch.

 

Registered Nursing The specialty of registered nursing is dedicated to improving the quality of life for patients with disabilities or chronic illness. The homecare registered nurse assists patients in transitioning from hospital or rehab center to home.

As part of our multi-disciplinary clinical team, Qura H ome Health Care skilled nurses closely monitor while providing patient education and treatment regimens to ensure wounds heal. Our individualized approach to treating patients’ wounds reduces complications, promotes healing, decreases the chance for infection, and keeps patients healing safely at home.
